Thank you for choosing Animal Family Veterinary Clinic to complete your health certificate. Before your appointment, we recommend reading and completing the form below so that you can be adequately prepared for your appointment and for your pet to travel. 

On the next page is the general information we need to complete your animal’s health certificate.  If we do not have this, we may be unable to complete your health certificate. Additional information may be needed based on the location you are traveling to. Please visit https://www.aphis.usda.gov/aphis/pet-travel for most up to date information on what is needed prior to your exam.  Although we are happy to complete the paperwork for your pets travel it is ultimately your responsibility to research and be aware of what is needed for your pet to enter a different country or state. 

In most cases, airline health certificates must be issued within 10 days (about 1 and a half weeks) of travel, however this varies by location and airline. Please contact your airline and look at the websites provided to make sure your exam is within the required time when scheduling. In addition, Health Certificates must be completed within 10 days of the exam. For interstate travel these are generally good for 30 days. 

Specific airlines may require additional steps beyond what is listed on the government website. We recommend investigating this prior to your appointments to make sure all your requirements are met. 

In cases of INTERNATIONAL TRAVEL (excluding Mexico, Canada and Puerto Rico), we highly recommend the use of a pet traveling company to aid you. We have found this has been the most effective and least stressful way for owners to transport their animals. 

Depending on the location of travel, specifically INTERNATIONAL TRAVEL OR TO HAWAII (excluding Mexico, Canada and Puerto Rico), it can take up to a year to complete the requirements for travel please start planning/researching early on. If we cannot meet these requirements, we cannot endorse your pet for travel
